HOLD HARMLESS, WAIVER, AND RELEASE AGREEMENT

Pop-Up Pack Walk – Mesa County, Colorado, Roice-Hurst Humane Society & Primary Care Partners

SECTION 1 — PARTICIPANT INFORMATION

SECTION 2 — EVENT DESCRIPTION

This Agreement applies to participation in the Pop-Up Pack Walk organized or sponsored by Roice-Hurst Humane Society (“RHHS”) and Primary Care Partners (“PCP”) in Mesa County, Colorado.(Required)

SECTION 3 — ASSUMPTION OF RISK

I acknowledge that interacting with and walking dogs involves inherent risks, including but not limited to bites, scratches, pulling, falls, allergic reactions, property damage, or unpredictable animal behavior. I voluntarily assume all risks, whether known or unknown.(Required)
In consideration for participation, I hereby release, waive, and discharge RHHS, PCP, and their volunteers, staff, officers, directors, agents, and insurers from any and all claims, including negligence, arising from my (or the minor’s) participation. This waiver does not release claims that cannot legally be waived under Colorado law.(Required)

SECTION 5 — INDEMNIFICATION / HOLD HARMLESS

I agree to defend, indemnify, and hold harmless RHHS and PCP from any claims or damages arising from: My actions or omissions; The actions of a minor in my care; Injury or damage to other individuals, property, or animals; Violation of event rules or applicable law(Required)

SECTION 6 — MEDICAL AUTHORIZATION

I authorize emergency medical care if necessary and accept financial responsibility for all associated costs.(Required)

SECTION 7 — PHOTO / MEDIA CONSENT (Optional)

I grant permission for RHHS and PCP to use photos or videos of me or the minor(s) for promotional or educational purposes.

SECTION 8 — MINOR / CHILD PARTICIPATION (Complete Only if Applicable)

Parent/Guardian agrees to the following additional terms:
8.1 Supervision & Responsibility: I certify that I am the parent or legal guardian of the minor(s) listed below and will supervise them at all times during the event. I understand that minors may interact with dogs and other participants in an uncontrolled outdoor environment.
8.2 Minor Assumption of Risk (Parent/Guardian Acceptance): I acknowledge and accept on behalf of the minor(s) the same risks listed above. I agree that the minor(s) may participate only under my direct supervision and that I accept all responsibility for their actions.

SECTION 9 — PARTICIPANT REPRESENTATIONS

I certify that: I (and any minor) am physically capable of participating; I will follow all instructions from RHHS staff/volunteers; I will handle animals only as directed(Required)

SECTION 10 — GOVERNING LAW & SEVERABILITY

This Agreement is governed by Colorado law. If any portion is found unenforceable, the remaining portions remain in effect.(Required)

SECTION 11 — ACKNOWLEDGMENT OF UNDERSTANDING

I have read and understand this Agreement and sign it voluntarily. I understand I am giving up substantial legal rights for myself and/or the minor(s).(Required)
