Signing over your dog

Instructions: Please fill out the following form with as much detail as possible. Please be as specific as possible when describing behavioral concerns. This information will assist us in creating an appropriate shelter training/enrichment plan, meeting your dog’s needs while in our care and finding it an appropriate home.

A fee of $100 per dog is required. At the time an appointment is made for you to bring your dog to the shelter, a $30 deposit may be required, which will be applied to the relinquishment fee when you arrive.

Please bring the following to your appointment:

Medical records
Any medications or supplements
Any of your dog’s belongings – blankets, beds, toys, etc.
A few servings of your dog’s food
Any forms not submitted online
If your dog was adopted from Roice-Hurst Humane Society, please bring the original adoption paperwork/folder

Submitting this form does not guarantee we will accept your dog into our adoption program. We will contact you for more information about your dog and to make an appointment. At the time of your appointment, we will perform a routine intake exam. If the dog shows signs of aggression that do not allow us to complete the exam, we will not be able to accept the dog.

Another option for finding a new home for your cat is Rehome, by Adopt-a-pet. This option connects you with people who are looking for a new pet and allows you to safely adopt your pet directly to another individual. If you are a resident of Mesa County, Mesa County Animal Services accepts dogs signed over by their owners. MCAS does not euthanize healthy, adoptable, or rehabilitatable dogs.
